Polk Senior Games provides an annual event of competitive athletic and recreational events for men and women 50 years and over. The Games promote an active, healthy lifestyle, an interest in sports, and volunteerism. The Games offer something for everyone - the serious competitor, the fun at heart, the seasoned athlete, and the physically challenged.

Polk Senior Games is a 501 (c)(3) non-profit organization, meaning it is designated by the IRS as a charitable cause, is exempt from federal income tax and can accept tax-deductible donations. Funding for the Games is raised through entry fees, sponsorships and donations.

ENTRY INSTRUCTIONS

Registration begins January 2010; Entry must be postmarked by February 11 or hand delivered by February 12, 2010.

Open to men and women 50 years and over. Polk County Residency is not required. Gold, silver and bronze medals are awarded in each event and age division.

$10 entry fee for first event, $3 fee for each additional event up to $28; additional fees for Bowling, Golf and team Sports

Events held throughout Polk County.

All participants receive a T-shirt specially designed by local artist Chryssie B. Tavrides.

An Entry Booklet with an entry form and complete rules and regulations is available upon request. Call 863-533-0055 or email polkseniorgames@verizon.net.

Each participant must submit an Individual Entry Form. If printing an entry form from the website, page 1 and page 2 must be printed, completed, signed and returned with the entry fee.

Age: The minimum age for participation is 50 years on or before December 31, 2009.

Events: Participants may enter as many events as their schedule allows with the following exceptions: Swimming – up to five events a day; tennis – up to two events. It is highly advised to be physically capable and familiar with a sport before competing. If an entrant demonstrates inadequate skill or physical incapacity for an event, for their safety or the safety of others, they will not be allowed to compete.

Fees: Enter the amount of entry fees according to the fee schedule. If playing 18 Hole Golf, Bowling or Shooting add the appropriate facility fee. If Bowling, add a $3 facility fee for each Bowling event entered. Total all fees. Team fees are not included in the Fee Box. Fees are non-refundable after the February 12th deadline.

Partners: Partners are required for most doubles events. Each partner must complete an Individual Entry Form. Names of partners must be included on the Individual Entry Form so rosters will be correctly matched. Please send the entry forms together, if possible.

Release of Liability: All participants must read and sign the Release of Liability on page 2 of the Individual Entry Form. If a Release is not signed, the entry will not be processed but returned to sender for signature.

Postmark: Entries must be postmarked by Wednesday, February 10th or hand-delivered to the Polk Senior Games office in Bartow by 5:00 PM on Friday, February 12th.

TEAM ENTRIES

Basketball 3 on 3, Softball, and Square Dancing

Each team member must submit a completed “Individual Entry Form” to be placed on the official team roster. This provides the information needed to send each player a confirmation letter, future Registration Books, name/address to Florida Senior Games as state qualifier, and provides a signed Release for each player. A player may use the same entry form to enter other events. Send fees for “other events” but not for the team event. The team manager sends the team fee.

The team manager must complete a team Entry Form and submit it with the fee. Please send the Team Entry Form, team fee and the player’s Individual Entry Forms together, if possible.
