Polk Senior Games provides an annual event of competitive athletic and recreational events for men and women 50 years and over. The Games promote an active, healthy lifestyle, an interest in sports, and volunteerism. The Games offer something for everyone - the serious competitor, the fun at heart, the seasoned athlete, and the physically challenged

Polk Senior Games is a 501 (c)(3) non-profit organization, meaning it is designated by the IRS as a charitable cause, is exempt from federal income tax and can accept tax-deductible donations. Funding for the Games is raised through entry fees, sponsorships and donations.

Entries must be postmarked by Wednesday, February 8 or delivered to the Polk Senior Games office in Bartow by:
5:00 PM on Friday, February 10.

INDIVIDUAL ENTRY INSTRUCTIONS
Print Individual Entry Form AND Release of Liability

Each participant must submit an Individual Entry Form and Release of Liability

Complete all requested information, including age you WILL BE on DECEMBER 31, 2012

Check the events you wish to enter; check the schedule of events to avoid conflicts

Indicate names of partners for each partner event entered so players will be correctly matched

Partners should send entry forms together, if possible

Indicate the Team Name in the Team Events box if playing Softball, Basketball 3 on 3, and/or Square Dancing

Complete the Fee Information Box. Enter the amount of entry fees according to the fee schedule

Add the Additional Fees if applicable. Bowling, 18-hole Golf, Trap and Skeet Shooting

Total all fees and include check or money order

Send information sheet if entering Talent Show

Read and sign the Release of Liability; unsigned forms will NOT be processed

TEAM ENTRY INSTRUCTIONS
Team Managers for Basketball 3 on 3, Softball, Square Dancing Print and complete a Team Entry Form

Print a Release of Liability - Make sure each member, playing or non-playing, reads the Release of Liability and signs the Team Entry Form Roster in agreement

Send the team fee and Team Entry Form including all players’ signatures, ages and shirt sizes.     

Team Members playing ONLY on a team
All members, playing and non-playing, must read the Release of Liability and sign the team roster in agreement.

Team Members entering events other than team sports:
If playing in other events as well as on a team, complete an Individual Entry Form (see above instructions).
